The Role

WEEKEND ONLY

Care at Home are currently looking to recruit a Care Coordinator to join our team. We are a well established home care provider who want to make a difference in people's lives in the community and support people to stay in their own homes for longer. Your main areas and responsibilities are based around the staff and service.

You will be responsible for providing overall coordination of staffing and service user rota's to ensure service provision is met as stated from commissioners and in line with staff availability. Liaise and work with Registered Manager, Directors and Operations Lead to ensure companies' policies are adhered too and systems are kept updated.

PLANNING/ ALLOCATION

  • Ensure all customer details are entered on to the system accurately prior to service commencement
  • Enter and maintain staff availability on to system and is adhered to
  • Allocate all customer visits in line with their commissioned /agreed times
  • Communicate any changes to planned times to both customer and staff member
  • Updated rotas sent to avoid any missed calls
  • Ensure staffing rota's are distributed on a weekly basis
  • Plan and arrange shadowing for newly recruited workers in the absence of the recruiter.

CUSTOMER LIASION

  • Identify key contacts within commissioning bodies and maintain regular weekly contact advising of availability for new care packages.
  • Ensure all increases, decreases and cessation of packages are accurately documented and accurately adjusted on Carefree
  • Assist with monitoring service user satisfaction and all quality assurance issues as required
  • At all times present a professional and caring image of Care at Home and demonstrate behaviours reflective of Care at Home's values.
  • Take a proactive approach in increasing hours and sourcing new packages of care

SERVICE DELIVERY

  • Report all care worker conduct, absence, grievance, and capability issues to the Registered manager.
  • Deal with any level 2 complaints in accordance with complaints policy.
  • Communicate regularly and maintain professional working relationships with all office and field-based staff
  • Manage annual leave requests from care workers to ensure service delivery is covered and at no time compromised by absence.
  • Carry out visits to service users in the event calls are not covered.
  • Participate in staff meetings and service user meetings as required.
  • Take part in on call out of hours rota / on call back up

REQUIREMENTS FOR CARE-CO'ORDINATION

  • Hold a FULL UK Driving Licence
  • Have Flexible working hours
  • Excellent time management
  • Willing to work towards NVQ Level 3 Upwards
  • Essential 2 years care experience
  • Good communication skills
  • Reliable and Punctual
  • Work well as a team or alone
