The Role
Key Responsibilities
- Act at all times within own competence and knowledge in line with Nursing and Midwifery Council (NMC) requirements.
- To deliver high quality care focused on residents individually assessed needs and choices.
- To ensure the ongoing safety of residents, staff and visitors through systematic assessment, monitoring and reviewing processes.
- To effectively conduct and document care/risk assessments, in addition to developing care plans related to the assessed needs and choices of individual residents.
- To review the daily delivery of care, including pressure area prevention, nutrition and infection prevention and control procedures. Ensure high standards of care delivery are maintained.
- To ensure that areas of identified risk are appropriately monitored, and outcomes recorded and acted upon e.g. fluid and nutrition charts, repositioning regimes, blood glucose monitoring.
- To safely and appropriately ensure the management, administration and recording of medication, including controlled drugs and oxygen, are in accordance with NMC requirements and Acacia Policies and Procedures.
- Liaise with the General Manager/Clinical Lead and GPs/other health professionals to ensure that residents receive the medical and clinical support they require.
- Be knowledgeable about Safeguarding Vulnerable Adults Policies and Procedures and report any untoward events in accordance with Acacia and Local Authority requirements, (ensure requirements are incorporated into practice).
- Understand the requirements of the Mental Capacity Act (2005) and Deprivation of Liberty Safeguards (2008) in relation to consent and best interest decision making.
- Report all accidents and incidents promptly in accordance with Acacia Policy. Ensure accident/incident reports are written clearly and concisely and that any remedial action or learning is incorporated into care delivery ensure requirements are incorporated into practice.
- To make certain clear and accurate records and communication systems relating to clinical and care delivery are maintained and effectively used.
- To provide a high standard of record keeping in accordance with NMC, Health and Social Care Act (2008) and Essential Standards of Quality and Safety (2010) and company requirements.
- To ensure that comprehensive, individualised care plans are in place which are regularly reviewed and evaluated in accordance with the NMC and Acacia policy.
- To assist and deal with emergencies and pre-planned treatments of patients.
- To be responsible for effectively supervising the care team and their rotas.
- To ensure that all care practices within the home are maintained to the highest levels and that all documentation is in line with current regulatory framework.
- To administer medication, surgical dressings, catheterisation, tube feeding and any other relevant required nursing task.
- To provide advice on nursing care and disease prevention.
- To provide advice to families of residents and GP's of any change in conditions which give rise to concern.
- To develop and maintain positive working relationships with all residents, families, visitors and colleagues.
Relevant facts and figures relating to the job
- To provide leadership and support for care staff and act as a champion for residents' safety and dignity.
- Supervise care staff on a day to day basis and through formal supervision processes.
- Maintain standards of infection control within the home and to assist residents to maintain their own infection control needs.
- Attend and participate in nurse and staff meetings as arranged by the Home Manager/Clinical Lead.
Key measures
- Maintain skills and keep up to date with clinical developments and best practice in areas relevant to care delivery in the home and to comply with NMC validation.
- To undertake training appropriate to the role, as agreed with the Home Manager, and ensure learning is transferred into practice.
- Participate in continuous professional development to ensure requirements of the NMC.
Person specification
- To have an active NMC pin.
- To have prior relevant experience as a Nurse.
- To have excellent communication skills
- To have the relevant skills and experience for the role.
- To have adequate knowledge and understanding of the job role.
- To be good under pressure and be able to work to deadlines.
- The ability to work well with others.
